Replacing the wires in the nespi case to larger wires is the fix for the nespi cases. Running a hard drive without its own power is pretty power hungry.

So i need to give up on the case ??
i didnt really get it .
so if i will put the usb 1tg it will always give me the light yellow bolt ???
cause in a plastic case everything works fine.

not with the usb the have in the case itself ???

@shavecat Are you trying to power the USB drive off the PI ? That's not going to work - you need to power the USB drive externally - no matter what case you have.
If you want to see if the case is faulty, just remove the PI from it and see if you still get the low voltage warning with other devices connected. As the other said already, make sure you're using a proper power source for the Pi. -

with the hard drive usb, no lighting bolt ever .
will try to buy power adapter and check again.

In this case, I think the answer is obvious. As I said, it's a simple test you can do to see if the case is at fault - remove the PI from the case and try to see if you get the under-voltage sign. If you still get it, then it's the case, not the power supply.

"are u saying i need to connect the hard drive itself to the raspberry pi itself inside the case" - I wouldn't recommend doing that for heat reasons if it is even possible, I am using a thumb drive and it barely fits.
"not with the usb the have in the case itself ???" I run the thumb drive off the internal usb. I only use the external usb ports for keyboard mouse and gamepads, but it does power a usb drive without lightening bolts.

The easiest thing to do would be examine the wiring on the usb hub on the nespi case, you can either fix the existing wire with a soldering iron or just replace the wiring all together. Honestly the best solution may be to just take out the nespi's usb hub and wire a direct usb extention to the raspberry pi's usb connectors; that is what I have been planning on doing to mine since the usb hub is just a liability and it would make more room inside.
"So i need to give up on the case ??" - Not at all! it is a great case, looks just like an OG NES and has plenty of room for goodies inside. I knew the wiring was a problem when I bought mine but I was planning to customize it anyway. For the price it is pretty good, but it is fair to expect the wiring to be adequate.

I'm going to buy a new powersupply, to upgrade from the aftermarket 5V 2.5A, and hopefully alleviate the low voltage while charging a controller.
Maybe I can recycle my old Power supply, it works for other models, I don't know what else.
I'll share the results when I get the new one.Question though. I have a different model case, the megadrive case. How do I get the fan inside to stop being so noisy? It is like it is being pressed down on, and I've tried loosening the screws. Is there any tip or trick one could advise?

It isn't its normal function, something presses down on it and it rattles. If I shake it a little bit sometimes it will set right.I do like that fan controller though. It would be able to shutdown the fan on OS poweroff.
Oh yes, and an obligatory, always remember to remove the sdcard when disassembling and assembling a case.

@Efriim does it make the same noise if the case is left open? Or just when you screw it back together? Maybe you need a thinner fan.

When I screw it back together! I think it does it if the fan screws are too tight also. More like a whine, yeah it's too tight....It was too loose, I tightened the screws, they turn hard and their long so its difficult to judge how in they are. Only needed to mess with it some more.
It will probably start doing it again, sort of why I gave up to avoid being frustrated with it. -
