I am running Retropie 4.5 on the 3B+ and am running roms from an external USB. I am wondering if you can change the directory for cheats to the USB so I can edit them as easily as adding more roms.

There's an option called cheat_database_path in the retroarch.cfg. It points to the cheats directory by default. I have not actual experience in changing it, but this sounds like the thing you're looking for.

@Clyde yes, I've heard of this but I'm not sure what to change the directory to to point at my external USB.

Normally, a usb drive is mounted as /media/usb0/. Just plug it in and enter mount in the RetroPie console (F4 in Emulation Station). The drive should be one of the last entries listed. You can also filter the output for lines that contain "usb" by mount | grep usb.

Create a cheats folder under roms (same level as each system's folder), then change the RetroArch settings to point the cheat folder to /home/pi/RetroPie/roms/cheats.
