I have a bartop arcade that I built. It is running on a Pi3b+ with 2 two players with 6 buttons, 4 admin button and a coin and player start button for each.

I have recently upgraded to a pi 4 2 GB and started with a pre-canned image that I got online. Despite anything I try I can't map the buttons correctly. Particularly the coin and player start buttons. Anyone else running into this issue?

Third-party images are not supported. Remapping works on the official image, by the method described in the official docs. If it doesn't work by that method on your pirate image, that's because something has been altered. We don't know how the person who made your image has reconfigured it, so we can't tell you how to fix it. Ask them for help.

Or use the official image and remap your buttons with Core Input Remapping:

@sleve_mcdichael I originally tried the standard image and ran into the same issue. I will try again with a fresh install from the site and post the result on here.

@jeepdrvr if you can reproduce the issue on the standard image we can help you troubleshoot it there. A potential solution might, or might not, port over to the custom image (you'll be on your own trying it there, though.)
