I recently got this controller and was having weird issues until I found the [X - D] switch (xbox/dualshock) on the bottom of the controller and re-setup the configs with the controller in Dualshock mode. No more problems.

The 'X' stands for "XInput" and the 'D' stands for "DirectInput".
