No boot after shutdown.

Hello!
I am having a bit of a problem with my newest RPI build. I am using the Raspberry Pi 3, model B with a cana kit case, 2.5v power supply, and heat sinks. I have build 4.2 flashed from the premade image. I have 2 wireless PS3 controllers and a usb keyboard connected. I have added games, configured my wifi controllers, etc.
Everything works great, on the first boot.
As soon as I turn the system off and try to turn it back on, it refuses to boot. I have tried shutting it down through the terminal, the emulationstation menu, and the bad way (just unplugging it) and I always get the same issue when I try to turn it back on. No boot.
Now, the weird thing is after I have turned it on and there is no boot I can go through WinSCP and go to the terminal and sudo reboot, and it restarts the RPI3 again and everything comes back up just like I left it. No harm, no foul.
I thought it could be an HDMI issue. I am using an hdmi/vga adapter. But no amount of changes to the config.txt file gets me anywhere. I also thought it could be an SD card issue. Tried 3 brand new ones from different companies. No luck, same issue.
Anyone have this experience?

i had a defective HDMI/VGA adapter once....one wire inside was loose and maybe shorted something inside......
could you test the RPI on a HDMI monitor? (without the HDMI/VGA adapter)

Can you try the original image from this site, and see what it does after a shutdown?
Maybe it's a faulty image you found. -

That was it! Thank you so much. For some reason it would always boot the first time, but never after that. Changed it to straight HDMI with a new cable and it has been rock solid sense. -
