Just old well known retro-games are still pretty hard to beat.
Take classics like Street of rage, super mario, golden axe, megaman, metroid series...
I don't think someone that has always known autoheal and infinite continues will do so well, at least the first few tries :) -
I recently discovered the most brutally hard game I could ever imagine. Nothing comes close.

First, patch the rom with this: https://www.romhacking.net/hacks/3875/
(game is unwinnable without it)Second, I'll say that the game has good music, voices and graphics. But my praise ends there. The best way to describe this game is if someone wanted to rip off Sonic but failed poorly. Like, you go fast as all f**k and thats it. The levels and enemies were not designed for high speed platforming. You will find yourself running into enemies and off cliffs.

Dreaming reminds me of Weird Dreams. I played it on the Commodore Amiga back then. Although "only" some kind of adventure game, I remember dying repeatedly. This game about a nightmare one has to escape from was a literal nightmare for me, I even remember dreaming about it one or two times.
So, apart from its high difficulty I remember, its creepy setting and atmosphere would be "perfect" to give a mouthy child something to fear … 😈

Also there's Shadow of the Beast (the first one) on Commodore Amiga. That game is really tough!

Ren & Stimpy Veediots and Ren & Stimpy Time Warp were probably the hardest games I had on my SNES. I vaguely recall Stimpy's Inventory being equally as hard.

I was thinking about both pitfalls for the 2600 or msx. Other games you should let him play are the Ninja Gaiden games for Nes and Mastersystem :D

Any of the SNES 'Super' Star Wars, Empire Strikes Back was very very hard, Smash TV Could be tricky, sure that was just all out 'Total Carnage', I recall firing up some of the good old Shooters, something like 'Flying Shark' I failed badly despite loads of credits pumping in.
Oh, Mike Tysons Punch Out on the NES...
Pretty much anything before we got mid level checkpoints :D I'm not the greatest at FPS, but beat one of the CODS on Veteran on the 360 in under 14 hours!

I think Batman Returns on the Atari Lynx is still possibly the hardest game I've ever played. Couldn't tell you what the game is like past level 2

The first level of that game is really the only one worth playing anyway. In my opinion, it's one of the most inventive you'll find throughout the entire NES library. However, it's almost as if the developers gave up moving forward, as every other level and sequel is your basic shovelware concept for generic platformers of collecting items and beating an end boss. I would have loved to see a whole game patterned after that first stage.
