@bifelix89 so you dont have a sorta secret button where a right trigger/shoulder button would be? What controller do you have?

I'd recommend the Hyperkin GN6 (either USB or standard Genesis connector with a mayflash adaptor) for Genesis/Megadrive.
Feels very similar to the original 6 button controller and has the mode button on the right shoulder which can be used as the hot key/select.

@bifelix89 yep... unfortunately, it looks like it would be best to have one more button.

Sure, but answering the question: you can at any button to the hotkey, so in your case if you don't have a select button, you can probably use L1 or R1 though you'd need to edit RetroArch config so that it doesn't also use that key for its original intent (load or save state).

That being said, I echo some of the comments here. It's been 15 years or so but I'm under the impression that there was a mode button on my 6 button controller.

I wonder if there would be a place to solder an additional button onto the pcb inside the controller?
those controllers are the same as the ones I got with my AT games megadrive, not very good in my opinion.
I would open it and see if you can add another button personally, and if not I would probably replace it.

@bifelix89 set the "a" button as your hotkey in emulationstation start menu controller configuration. Just so the hotkey option gets generated. Now open up the file /opt/retropie/configs/all/retroarch/autoconfig/NameOfYourController.cfg . Find the part for exit and hotkey. Assign the number that is your start key to the hotkey and change the exit to another key. (Not b or x as they are used for other options)
