No input accepted on welcome screen (KB/Controller)

I've installed the 4.1 RetroPi image from your site. Attached a USB keyboard/HDMI then booted.
I get to the welcome screen saying no gamepads detected, that's fine. At this point I go to hold down a key to create a profile for the keyboard and nothing happens. Tried several keys. The keyboard does work as when the Pi is booting you can mash keys and they'll show up. Hitting F4 at this stage also doesn't drop back to the terminal.
When using a controller the same happens. However it says '1 Gamepad detected' and holding down any of the buttons for several seconds will not cause anything to happen.
I've tried with a DS4, Mad Catz USB controller, several power supplies, a 1920x1080 monitor, a 720p TV even a 3.5" composite LCD screen and all provide the same results, stuck on the welcome screen.
Also tried installing the image several times over after verifying the checksums.
Keeping the same keyboard setup and booting into an OS such as Raspbian allows me to use the keyboard as expected.

Power Supply used: Tried both a USB3.0 and 1.5A Power Supply
RetroPie Version Used: 4.1

USB Devices connected: 1x Keyboard / 1x Controller (not at the same time, was testing individually)
Controller used: Mad Catz Xbox 360 lookalike (this has worked with retropie in the past), also tried DS4

How to replicate the problem:1.) Load a 4.1 image on a Raspberry Pi Zero
2.) Plug in keyboard
3.) Turn on monitor
4.) Boot
5.) After loading the OS there's no way to provide any form of input through controller or keyboard -
@adzter My first advice will always be to re-flash the image on the card. If you are not too far into setting things up then this is usually the best option. Sometimes things just don't work and a fresh start can sort it.

@AlexMurphy I've re-installed the image several times after verifying checksums, same issue.

@adzter Are you using a powered USB hub to connect the devices via OTG? How are you powering the PiZero?

@AlexMurphy I've tried powering the PiZero through a 1.5A Power Supply connected to the mains and a USB 3.0 port on my PC. Same issue on both. Note that with Raspbian running both of these work for powering the keyboard and it works as expected AND when RetroPie is powering up I can hit a few keys and it'll show in the terminal as it boots. The keyboard works.
Not using any form of USB hub just OTG -> Keyboard.

@adzter Neither the KB or controller would draw so much power to cause the issue. Not if you are powering the zero via 5V 1.5A supply. Avoid the USB power option it only complicates things! Have you any other elements in your setup that are personal to you or custom?

@AlexMurphy Just to be certain I've attempted with a powered USB hub, same issue occurs. It says I've got a controller connected but can't do anything with it, no input again. Same with the keyboard
There's no parts custom or personal, all standard leads/cables where there are any.

@adzter Even I have exactly the same error that you have. Same setup and the same sd card. It could be something wrong with the sd-card. I'll get a new one and test.

I'm going to build the latest release from Github and try that, if that doesn't work i'll begin to pick through the source code until I find the issue.

When attempting to compile using Debian I was getting kernel panics and references to null pointers in memory. After trying a different SD Card, Cables, and power sources everything has only began to work once I've bought a new Pi.
I can only assume I've got a faulty unit?
Regardless I'm now able to run RetroPie on the exact same setup (with the Pi swapped out).
